(“SkinTox,” “Company,” “we,” “us,” or “our”)
By booking, scheduling, or receiving services from SkinTox Medical Spa & IV
Hydration Bar LLC, you (“Client,” “you,” or “your”) agree to the following Terms
and Conditions.

These Terms constitute a legally binding agreement between
you and SkinTox Medical Spa & IV Hydration Bar LLC.

If you do not agree, you
must not use our services.

1. ELIGIBILITY FOR SERVICES
YOU MUST BE AT LEAST 18 YEARS OLD TO RECEIVE OUR SERVICES.

All clients must:
 Complete all required intake forms truthfully and accurately
 Disclose full medical history, medications, allergies, and prior reactions
 Participate in a Good Faith Exam (GFE) when required

Services are nurse-led and physician-supervised. A licensed medical provider
(Medical Doctor or Advanced Practice Provider) will evaluate eligibility for
treatment based on medical history and examination findings. Final approval of
treatment is subject to medical clearance.

Repeat Clients:
A Good Faith Exam (GFE) is valid for one (1) year unless significant medical
changes occur.
New Clients:
A GFE is required prior to receiving any medical treatment.
We reserve the right to refuse service at our sole discretion if treatment is
deemed unsafe, inappropriate, or medically contraindicated.

2. MEDICAL DISCLAIMER & RISK ACKNOWLEDGMENT
OUR SERVICES ARE NOT A SUBSTITUTE FOR PRIMARY OR EMERGENCY
MEDICAL CARE.
SkinTox provides elective wellness and aesthetic treatments, including but not
limited to:
 IV hydration therapy
 IM injections
 Vitamin therapy
 Aesthetic injectables
 Wellness treatments
We do not diagnose, treat, or cure disease.

You acknowledge that all medical treatments carry potential risks, including but
not limited to:
 Bruising
 Infection
 Allergic reactions
 Vein irritation
 Swelling
 Pain
 Dizziness
 Nausea
 Fluid overload
 Vascular complications (for injectable treatments)

YOU AGREE TO DISCLOSE ALL RELEVANT MEDICAL INFORMATION.

Failure to do so releases SkinTox from liability for complications resulting from
undisclosed conditions.
Results vary by individual. We make no guarantees regarding specific outcomes.

Medical Disclaimer:
Statements regarding vitamins, supplements, or therapies have not been
evaluated by the Food & Drug Administration. These services are not intended to
diagnose, treat, cure, or prevent any disease.

3. BOOKING, PAYMENT; REFUND POLICY
All appointments are subject to availability.
Appointments may require a deposit to secure booking. The remaining balance is
due prior to or on the day of service.

Accepted payment methods may include:
 Credit/Debit Card
 Apple Pay
 Google Pay
 Cash (if applicable)

PRICES ARE SUBJECT TO CHANGE WITHOUT NOTICE.
Cancellations
 Cancellations 24+ hours before appointment: Eligible for rescheduling or deposit
credit
 Cancellations under 24 hours: Deposit is forfeited
 No-shows: Deposit is forfeited
Refunds are not provided once services have been rendered.
Consultation fees, travel fees (for mobile services), and telehealth fees are non-
refundable.

4. PRIVACY & HIPAA COMPLIANCE
SkinTox Medical Spa & IV Hydration Bar LLC complies with:
 The Health Insurance Portability and Accountability Act (HIPAA)
 Applicable Virginia state privacy laws
All medical records and personal information are securely stored and accessible
only to authorized personnel.
We do not sell personal data.
Clients have the right to:
 Request access to medical records
 Request corrections
 Be informed about data usage
Requests must be submitted in writing.

5. CLIENT RESPONSIBILITIES
Clients must:
 Follow all pre- and post-care instructions
 Report unusual symptoms immediately
 Attend follow-up appointments when advised
Failure to follow instructions may increase risks and impact results.

6. LIMITATION OF LIABILITY
ASSUMPTION OF RISK
By receiving services, you acknowledge and voluntarily assume all inherent risks
associated with medical and aesthetic treatments.

RELEASE OF LIABILITY
To the fullest extent permitted by Virginia law, you release and hold harmless
SkinTox Medical Spa & IV Hydration Bar LLC, its medical director, employees,
contractors, agents, and affiliates from any claims arising from:
 Complications
 Adverse reactions
 Infections
 Allergic responses
 Dissatisfaction with results
This release includes claims based on negligence except where prohibited by
law.

INDEMNIFICATION
You agree to indemnify and defend SkinTox against claims resulting from:
 Failure to disclose medical history
 Non-compliance with instructions
 Misuse of services

NO GUARANTEE OF RESULTS
We do not guarantee specific outcomes.

GOVERNING LAW
These Terms shall be governed by the laws of the Commonwealth of Virginia.

SEVERABILITY
If any provision is found unenforceable, remaining provisions remain valid.

NON-DISPARAGEMENT
Clients agree not to publish knowingly false or defamatory statements regarding
the Company.

7. MARKETING & MEDIA CONSENT

Marketing use of photographs, testimonials, or reviews requires written consent.
We will not disclose medical information without authorization.You may opt out in writing at any time.

8. TELEHEALTH CONSENT
You acknowledge that telehealth consultations may be conducted by an
Advanced Practice Provider or Physician to determine eligibility for treatment.

You consent to:
 Electronic communication
 Remote evaluation
 Electronic documentation
Final treatment approval remains under physician oversight.

9. TERMINATION OF SERVICES
SkinTox reserves the right to terminate or refuse service due to:
 Inappropriate behavior
 Fraud
 Non-compliance with medical guidance
 Safety concerns
No refunds will be issued for services terminated due to policy violations.

10. SELF-PAY POLICY
All services are self-pay.
We do not bill insurance and do not guarantee reimbursement from third-party payers.

11. FORCE MAJEURE

SkinTox shall not be liable for delays or cancellations caused by events beyond
our control, including natural disasters, government actions, pandemics, or
emergencies.

12. INTELLECTUAL PROPERTY
All website content, logos, branding, text, graphics, and materials are the
intellectual property of SkinTox Medical Spa & IV Hydration Bar LLC.
Unauthorized reproduction or distribution is prohibited.

Client Acknowledgment
By booking or receiving services from SkinTox Medical Spa & IV Hydration Bar
LLC, you acknowledge that you have read, understood, and agreed to these
Terms and Conditions.
