Welcome to Skin Tox Medical Spa & Hydration Bar (“Company,” “we,” “us,” or “our”). By booking or receiving services from us, you (“Client,” “you,” or “your”) agree to the following Terms and Conditions. These terms form a legally binding agreement. If you do not agree, you must not use our services.

 

1. Eligibility for Services

You must be at least 18 years old to receive IV therapy and most injectable treatments unless otherwise permitted by law and approved by a licensed medical provider.

All clients must complete medical intake forms truthfully and accurately prior to treatment. Providing false, incomplete, or misleading information may result in denial of service.

A licensed medical provider will assess eligibility for IV therapy, vitamin injections, or aesthetic treatments based on your medical history and evaluation. We reserve the right to refuse service at our sole discretion if treatment is deemed unsafe or inappropriate.

Repeat clients may require updated medical clearance if health conditions change.

 

2. Medical Disclaimer & Risk Acknowledgment

Our services—including IV hydration, vitamin therapy, dermal fillers, neuromodulators, and other aesthetic treatments—are elective wellness and cosmetic services.

We do not diagnose, treat, cure, or prevent medical conditions.

You acknowledge that treatments may carry risks, including but not limited to:

  • Bruising

  • Swelling

  • Infection

  • Allergic reactions

  • Vein irritation

  • Dizziness

  • Nausea

  • Fluid overload

  • Injection-site complications

You agree to disclose all medical conditions, medications, allergies, and prior reactions before treatment. Failure to do so releases Skin Tox Medical Spa & Hydation Bar from liability related to undisclosed information.

Results vary by individual. We do not guarantee specific outcomes.

If you are experiencing a medical emergency, call 911 immediately. Our services are not emergency medical care.

 

3. Booking, Payments & Refund Policy

Appointments are subject to availability.

A deposit may be required to secure your appointment. Remaining balances are due prior to or at the time of service.

We accept approved forms of payment including credit/debit cards and other listed payment methods. Prices are subject to change without notice.

 

Cancellations & Refunds

  • Cancellations made at least 24 hours in advance may be rescheduled or refunded (deposit policy may apply).

  • Cancellations with less than 24 hours’ notice may forfeit the deposit.

  • No-shows may forfeit deposits.

  • Refunds are not provided after services have been rendered.

  • Consultation fees and travel fees (if applicable) are non-refundable.

If a client is deemed medically ineligible prior to treatment, a refund may be issued at our discretion.

 

4. Privacy & Confidentiality

We are committed to protecting your personal and medical information in accordance with applicable privacy laws.

Client records are stored securely and accessed only by authorized personnel for treatment and operational purposes. We do not sell personal data.

Clients may request access to or correction of their personal information by contacting us directly.

 

5. Client Responsibilities

Clients agree to:

  • Follow all pre- and post-care instructions

  • Arrive on time for appointments

  • Communicate respectfully with staff

  • Provide accurate and complete medical information

Failure to follow instructions may impact results and increase risk.

 

6. Limitation of Liability

By receiving services, you acknowledge and voluntarily assume all inherent risks associated with elective IV and aesthetic treatments.

To the fullest extent permitted by law, Skin Tox Medical Spa & Hydration Bar, its owners, providers, employees, contractors, and affiliates shall not be liable for any indirect, incidental, or consequential damages arising from services provided.

We make no guarantees regarding results.

If any portion of these terms is deemed unenforceable, the remaining provisions remain in effect.

 

7. Marketing & Testimonial Consent

With your written consent, we may use testimonials, reviews, or images for marketing purposes.

We will not disclose personal medical information without explicit authorization.

You may opt out of marketing use in writing at any time.

 

8. Code of Conduct

Clients are expected to behave respectfully. Abusive, threatening, or inappropriate behavior may result in refusal or termination of services without refund.

9. No Insurance Coverage

Our services are self-pay. We do not bill insurance or guarantee reimbursement.

Clients are responsible for all treatment costs.

 

10. Force Majeure

We are not liable for delays or cancellations due to circumstances beyond our control, including natural disasters, government actions, pandemics, or power outages. Appointments may be rescheduled or refunded at our discretion.

 

11. Intellectual Property

All website content, branding, text, graphics, and service descriptions are the property of Skin Tox Medical Spa & Hydration Bar and may not be copied or reproduced without written permission.

 

12. Modifications

We reserve the right to update these Terms & Conditions at any time. Continued use of our services constitutes acceptance of any changes.
